The Transition Management toolkit is designed to enable organizations to understand different facets of the transition of services from the buyer to the supplier. The toolkit facilitates the design of the transition process and provides sample documents that can be leveraged as the starting point to initiate a transition.
The tools and templates in the Transition Management Toolkit serve as excellent starting points to quickly and effectively plan, design, and train stakeholders to effectively manage outsourcing transitions from the buyer to the supplier of services. It is not a one-size-fits-all solution and will require a varying degree of customization based upon the user’s specific needs. In addition, it is not a replacement for having experienced resources involved in the transition process.

The document also attempts to illustrate, at a high level, how these phases play out in the outsourcing of different processes.
Transition framework document illustrates the transition framework within the overall lifecycle of managing complex outsourcing relationships. It provides clarity of roles, responsibilities, and key deliverables during the various phases of transition.
Transition checklist is intended to aid both service buyers and suppliers in developing a clear understanding of the transition process from a responsibilities/tasks perspective as early as possible. Moreover, it serves as a practical guide to construct a thorough transition plan, which must be in place during contract negotiations.
Transition dashboard is designed to provide an overview of the status of the transition at any point in time.
Transition job descriptions provide a sample of job descriptions relevant to an outsourcing transition. Each job description is comprised of: job summary, responsibilities, skills/competencies, and education/experience.
Integrated transition work plan provides a sample work plan for an outsourcing transition. This plan was developed for an HR outsourcing project. This is a sample document developed with the intention of showcasing the magnitude and rigor of a large outsourcing transition project plan.
